‘Organisational failure’: Safety risks at aerial firefighting operator

The aviation safety regulator found deficient procedures at Coulson Aviation, which operates the entire NSW aerial firefighting fleet, after the crash of a 737 water bombing aircraft. The company says it has fixed all the problems.
